Some people with EU Permanent Residence permits in NL are subject to inburgering and some are not.
You are exempt from inburgering if you have a permanent residence status based on Directive 2004/38 (the Belgium/EU route with a Dutch partner). Article 5, section 2(b) of the inburgering law.
You are not exempt from inburgering if you have a permanent residence status based on Directive 2003/109 (non-EU national with five years residence in another EU country), unless you have passed an integration exam in the other EU country. Article 5, section 2(c), which reads:
“ingevolge de wetgeving van een lidstaat van de EU of een andere staat die partij is bij de Overeenkomst betreffende de EER hebben voldaan aan een inburgeringsvereiste om de status van langdurig ingezetene in de zin van richtlijn 2003/109/EG van 25 november 2003 betreffende de status van langdurig ingezeten onderdanen van derde landen te verkrijge”

Your permanent residence does not expire every five years, there is a requirement to replace the card with a new pasfoto every five years.
