Enclosed the application form and information necessary to apply for a Certificate of Good Conduct.
The section '2. Data organisation/establishment' has to be filled out by the department that requires the certificate. If they are unable to do this, an official letter or brochure which states that you have to apply for a certificate of good behaviour will do as well.

For each certificate you must pay EUR 30,05. This sum must be paid upon submission of the application form.

Please make sure you have sent along a copy of your ID card or passport, as well as a copy of the payment with your applicationform. We will send you a VOG (Dutch Certificate of Good conduct) within 2 - 4 weeks. We will issue a Dutch certificate with an English summary.
Please note that cheques are not accepted.
