从本科转研究生改是哪种表格？

我跟ind要的是上面这个表格。。。不知道对不对。

I got the form from IND is called "Application for a regular residence permit without a provisional residence permit, or for a change to the purpose of residence"

俺也不知道。。。等高人来回答。。。是不是必须预约以后亲自到ind去申请change purpose。。。

但是order里面没有这个名字的表格，所以我要了个我说的那个，也许是一个表格，但是名称不同。
你说的这个是在下载选项里的，我这个是order的，ind会把表格寄到家。

In September 2007, the IND opened offices in Zwolle, Utrecht, Rijswijk, The Hague, Rotterdam, Eindhoven, Den Bosch, A'dam and Hoofdorp. You must go to the IND office to submit you application (changing the purpose of stay). You can no longer do this at the city hall where you live

when you make the appointment with IND, IND will sent a letter to you (includ the application form)

想问下从Ｈ转Ｕ是用哪种ＦＯＲＭ啊？都需要哪些文件啊？
