Copy of ALL writtenand stamped pages of your passport (if you were in The Netherlands before, youmust copy ALL pages of your passport);

The ORIGINAL filled out and signed RegularResidence permit application, please see the attachment (see 1-TEV Attachments Sep 2011);

The ORIGINAL photo card (see 1-TEV Attachments Sep 2011) including your passport photo and signature(Please see the attached 3-Passport photo guideline. It is very important that your photo meets the Dutch requirements!);

The Financing Study and Stay form, includingrequired financial documents (see 2-Financial Attachments Sep 2011)
红字标出的是三个附件 就是填写一下 都不麻烦~另外要拍一张荷兰的护照照 尺寸要求不太一样

别吵了，我发一下上海领事馆终结者版的邮件答复，任何文件都要去那里办理，他们说了才是最权威。

Now I have two pieces of seperated documents on hand: one is called original birth certificate which is issued by local hospital and the other is called notarial birth certificate which is issued by local notary office.
Which one should be double legalized? Thank you!

感觉还是不够清晰呢。。。如果是原件还是公证件需要被认证，那么显然是公证件啊。。。

那请问下 像我在上海上学 户口保留在学校 是回原户籍办理 还是直接在上海办理呢？

那就好了么，只要双认证那份出生公正书就可以了

不是这样的吧？好像应该是公证书上要加着源文件，源文件也是要被双认证的。目前的情况是国内的公证处不愿意加源文件啊。。。。

一来国内都是分开的，二来大使馆说双认证只要公证书就好了，人云亦云不如听大使馆的权威
