这个是我们学校给我的ID补办所需材料，LZ可以参考。应该是要先报案，申明ID丢失了。然后警察会给你一个报案证明。这个在补办ID的时候需要。
Checklist Replacement Residence Permit Card Pleaseinclude the following documents for the replacement of your residence permitcard: □ Copy of your passport – page(s) with personaldetails and period of validity only □ Originalextract from the Town Hall of Groningen which verifies your family situationand your home address – cannot be older than 6 months. □ Copyof police report (in case of theft or loss of RP card only); the closest Police station to the AcademyBuilding is located at Rademarkt 12. □ If applicable: if your RP card got lost or stolenabroad, you need a copy of two police reports: one from the country in whichthis happened, and one from the Police station in Groningen. It is important tokeep the original police report; only a copy is required for the application.If you pick up the new residence permit card, then you need to show theoriginal police report(s). □ Copyof both sides of current residence permit card (not needed in case of theft orloss) □ Passportphoto, please paste this on the document (Appendix Passport Photo Form) that isincluded in this package. The passport photo may not be older than 6 months andmust be made in the Netherlands. - Please be informed thatthe IND charges a fee of € 250,- for the replacement of the card.
