Documents to be send to me by post or pouch (hard copy)

-         signed attachment “Antecedents”;

* 2.2: fill in the place and date

* 2.3: sign the form with a black pen

The rest will be filled in by me

-         signed attachment “Photoform”;

* sign IN the box (bottom of the page on the left side; with BLACK pen)

* stick a passport size photograph on the grey space (right side

注意：自己的全名一般名+姓，而自己的签名比较随意，可以用姓+名，也可以用名+姓，中文签名相对更好一些，因为基本没有人能够模仿出中文签名。
