延ID填表的问题

老婆在这里陪读，去年7月份来的，前几天IND寄过来一封信，说是她的ID 8月份就要到期了，需要延的话就把附送的表格填好寄给他们。
我们在填表格的时候有些问题不太清楚，请教各位有经验的～～
第一个问题是，照片是不是必须再照啊，还能不能用去年申请ID的老照片啊？
第二个问题如下图，我用google翻译了一下，还是不懂，这个到底如何选啊，谢谢啦！！！

• the Act on Income Provisions for Older or Partially Disabled persons;
• The Work and Income artists;
• Supplements;
• Wajong;
• Temporary disability law limiting income impact criteria.

照片不能用老的，超过半年以上就不行了。我的理解是，没有拿过public refund就不用填的呗。
