Form B

今天收到学校发来的Form B，要求填写后和其它材料一并邮寄到荷兰，用于申请签证。
填的时候有点困惑，版搜无果，就来向各位前辈请教啦！




其中有Town/city、Date两栏，是指国内所在地和签名日期吗？
国内所在地是出生地，还是户口地呢？马上毕业了，正准备把户口迁回家。
最后一行：I submit this form and  ------------ (number) appendices 需要填吗？没有看懂……



底下的选项，发来的时候就有涂黑，那就什么都不用填了吧？

官方回答：

Q: Town/ city -- Does it mean where I'm from in China, or the city I'm going to live in the Netherlands?
A: The Chinese town that you live in.

Q: Date -- Does it mean the day when I sign this form, or the day when I arrive in the Netherlands?
A: The date when you sign the form.

Q: Do I need to fill in the last line: I submit this form and  ------------ (number) appendices.
A: If you don't add any other forms you can leave this blank.

第一个我们填的要去荷兰的那个城市，同学填完还给荷兰那边老师看了说没问题，到底应该填那个额

我今天也正要快递这个表格，也遇到了同样的问题，已经邮件去问了学校，不过还没收到答复。谢谢楼上的同学的回答。

我是6月16日在embark系统提交visa application，收到UvA Student Services自动回复的邮件“University of Amsterdam - Intake Procedure Submitted”。
第二天晚上，Immigration-StS先发来“Hardcopies photocard+Form B”的邮件，没过几分钟就收到UvA Student Services的“Form B”表格了。

额 我问我的学校说是填签字的时候所在的地点以及时间

啊！！到底该填那个额学校已经帮我在弄签证了，万一错了咋办
