The second form states that you have received a package from outside of the EU and that the Dutch custom agency cannot determine the contents and value of this package. They need that information in order to estimate the amount of taxes that you may need to pay.
So they ask that fill in the first form and return it in the enclosed envelope within 30 days and specify the contents and value. If you do not do that they will either send the package back or destroy it.
They also say you have to be truthful about the contents and value of the package. Otherwise you can get fine or something like that.
About the first form
- The first box is your personal information like name, phone number etc
- The second box should list the contents and VALUE (important) of the package
- The third box lists three options: Is it a:
* purchased item
* gift item
* you do not wish to accept the item
