我是一名文艺工作者，现在与荷兰公司签约前往荷兰参加演出工作，现在荷兰公司的申请的MVV 已经被批准，我发邮件询问使馆后，回了下面邮件。
我想问
1，我去办理签证的时候除了这些还需要其它文件吗？户口本需要全部复印吗？
2，我还需要支付其它费用吗？例如：签证费。
3，我是外地的，签证办理完后想让朋友帮取，如何操作？需要知道朋友的具体地址和身份证号码吗？
4，从进入使馆开始到出来大约需要多长时间？主要还得当天赶回去。
5，我应该填写哪个表格 Application form MVV (EN)还是Application form Schengen visa (EN/CH) ，表格上有所的问题都要回答吗，不能空吗？是否可以电脑输入，然后打印，手写签名？

You are requested to report to the embassy in order for your visa application to be processed. You have to make an appointment on our website to submit your application: www.hollandinchina.org.
