Checklist necessary documents for family reunion |

Application form.

2.
Copy of all the pages of the passport of the family member(s)

3.
Original excerpt from Town Hall (GBA) which clarifies your residence address and family situation.

4.
Original excerpt from Town Hall (GBA) which clarifies that your marriage was registered. Please note: numbers 3 and 4 will be combined in 1 excerpt.

5.
Copy of your valid residence permit (both sides)

6.
Copy of your passport (the page containing your personal data, date of issue & expiry).

7.
Copy of the marriage certificate (in original language + translation in English, Dutch, French or German, made by a sworn translator) provided with an Apostillestamp, or with a legalization stamp
In case your are not married: 2 declarations of not being married (one for you and one for your partner). These declarations need to be in the original language + a translation by a sworn translator provided with an Apostillestamp or with a legalization stamp. If the legalization procedure is applicable, marriage certificates and certificates of not being married need to be provided with the “Questionnaire for submission of foreign public documents” (to be obtained at the Dutch Embassy or Consulate General in your home-country).

Birth certificate of your child(ren) (in original language + translation in English, Dutch, French or German done by a sworn translator) provided with Apostillestamp, or with a legalization stamp. If the legalization procedure is applicable, the birth certificate needs to be provided with a “Questionnaire for submission of foreign public documents” (to be obtained at the Dutch Embassy or Consulate General in your home-country).
