I guess you refer to the Statement of Your Personal Details from the Town Hall of the city where you are currently staying.

不用，因为申请表上并没有要求，只要求你id，护照和合同的复印件。

yes, HR manager tells me the same story. but i saw the information in this website that GBA is necessary for the KM applyment, I hope it's not necessary, because I already send all the documents to IND.

很奇怪, 我第一年申请KM的时候, 好象也没交这个GBA, 不过今年续的时候, 就要求了
