UVA 的 bank slip!!!!!跪求前辈

You are your own sponsorIn this case the amount of € 9,240 has to be in your own bank account. You need to send a recent (not older than three months) bank slip (no fixed account) with your name on it, which shows the required amount of money. It is very important that the bank statement is stamped and has the following details printed on it: name, address and telephone number of the bank. 请问各位前辈这个bank slip到底怎么搞啊？是活期帐户每个月的流动底单？可是没用英文的啊？ 中国银行给开的中英文证明都是定期存款的但是上面明确说“no fixed account”？！请各位前辈指教这个证明到底怎么开啊。。。
