如何索要MVV申请表？

如题，有大虾建议网上写信索要，但本人已经在IND网站上contact栏by email发了两次邮件了，最早的一次已经两周了，只收到confirmation，但目前仍未收到回复或寄来的申请表。请问怎么才能尽快要到申请表？请有经验的大虾们指点迷津，不胜感激~~~焦急中~~~~

Many thanks for your advice. Is the no. 09001234561 ? But I can not get through this no. either by my mobile or my office tel., it is strange. Can I just call 09001234561 or plus any other no.? Thanks

是啊 这个电话说是0.1欧一分钟，但我打的不到3分钟就扣了3欧多~~~~~~~~~~不是说要申请表不要钱么？不明白，是够贵的

看来还得给钱才行，我写的邮件都两周了，毫无回音~~~打个电话花些钱，说5个工作日之内寄到
