I received positive decision from IND before new year.
You need the form: Request for advice preceding the actual application for a provisional residence permit (MVV). If you call IND and they will send to you by post. By the forn, you will find what documents you need, incl.:
- Your birth certificates.
- Copy of all the pages of your identity papers if you are a dutch nationality or of the front and back of your residence permit or of all of the pages of your passport in which the residence permist has been entered.
- an original GBA (or CBA) which you can buy from the city hall.
- proof of your income, incl. your contract, employer´s declaration, salary slips over the past 3 months, and evidence showing that your employer has reported your current employment agreement to the competent authority for paying national security contributions and taxes (ask your employer about this).
- If you pay for your parents, you also need to fill out the form of Guarantor Declaration.
